Communication and consent

This has the dual benefit of being able to provide a specific clinical role as well as a broader case management role which helps us to communicate directly and work closely with our colleagues in the health and social care sectors. With our client’s agreement we use these skills to keep all relevant parties informed of progress, including our client’s GP, employer(s), solicitors and insurance company claims handlers.

Forms for clients to sign

We have developed medical and employment mandate forms for clients to sign in order to show their consent to us having access to their medical information and their employment records, if appropriate. All treating doctors and health professionals require these forms before they can share medical information or discuss treatment options with us. They are therefore an essential in helping us progress a client’s rehabilitation. Generally speaking the OT/case manager will not visit a client without first receiving signed copies of these.

Sharing of information

We have a second consent form concerning the sharing of information, which we explain to clients at our first visit and which they can chose to sign or otherwise. The purpose of this is to gain their consent to make information about their rehabilitation available for any audits which are required by the Financial Services Authority and the Health and Care Professions Council, and may also allow  information sharing with colleagues for the purposes of supervision and quality assurance.
